Preparing for Your Visit

We look forward to your upcoming appointment at Arlington Dermatology. In order to coordinate your care safely and effectively, please complete and review the following:

Complete paperwork on our Patient Portal prior to your appointment:

  • You will receive an email from noreply@ezderm.com (our scheduling system) to confirm/verify your email address.  Once confirmed, you will be taken to the Sadio Patient Portal where you can create a unique username and password.
  • Prior to your appointment, please answer all intake questions including your pharmacy of choice, health history, current medications, etc.
  • Prior to your appointment, please sign all office consent forms.  You may locate them by clicking the “Appointments” tab at the top right of the browser page.  You can e-sign the consents.  This function is only available on laptops, desktops, and notebooks – NOT smartphones.  Please use an up-to-date browser Google Chrome and AVOID Internet explorer. 

You can access the patient portal at the following link. If you are a new patient, instructions for setting up your patient portal account can be found here. If you are an established patient, please review your information in the patient portal and make sure that it is up to date.

Please note our COVID-19 policy:

  • We ask that patients come to their appointments alone. Exceptions to our visitor policy include one adult visitor if needed for medical assistance, or one parent/guardian for minor patients. Patients and parents are asked to not bring along children unless the child is the patient being seen during the visit.  Exceptions may be granted for patients who require special assistance to access care.
  • Please call to reschedule if you develop symptoms of illness (even if you have had a negative COVID test). Symptoms include: fever, cough, sore throat, runny nose, loss of taste/smell, diarrhea, or body aches.

Please note for your skin examination:

  • Remove all make-up prior to your appointment
  • Do NOT wear lotion or moisturizer to your appointment

Bring insurance, ID, and form of payment to appointment:

  • Bring your ID and insurance card(s) to your appointment. A list of insurances we accept can be found here.  Please make sure to confirm with your insurance carrier prior to your visit that our office is in your network. Please also make sure to confirm with your insurance carrier if you need a referral prior to your visit, or you may need to be rescheduled if this is required and not present at the time of your appointment.
  • Payment is due at the time of your appointment. Depending on your insurance plan’s benefit, co-pay, co-insurance, or deductible costs may be included in the cost of your visit.
  • The office requires patients to have a credit card on file with Arlington Dermatology as part of our streamlined billing and collection process.  The card will only be charged for account balances that have not been settled within two weeks of billing.

No-show policy

In the event that you need to cancel or reschedule your medical appointment, please call 24 hours prior or a no-show fee will apply. If you need to cancel or reschedule your surgery or cosmetic appointment, please call 48 hours prior or a no-show fee will apply. We do this so your appointment slot can be offered to another patient in need of attention.
